[IASHRM] Staff Emergency Contact Information

Annually, we ask our employees to complete an emergency first-aid
information form, so that we have contact information for family members
or personal physicians and also an idea about hospital preferences, in
the event that an employee becomes ill or is injured while at work and
is unable to provide the information themselves.

 

As part of this, we would certainly like to know of any medical
conditions that an employee has.  I'm looking for some guidance on how
to obtain this information without violating anyone's privacy rights.
Any thoughts or best practices on this would be greatly appreciated!
