[IASHRM] Medical Insurance

I work for a small company that does cancer research and at this time we are
not generating any revenue.  Therefore, like everyone, controlling costs are
important.  My question has to do with Health Care.  I wondered if anyone
had any success stories or creative ideas for controlling costs.  I was also
wondering if anyone had done much with consumer driven health care and
educating employees in what they can do to control costs.  Additionally,
does anyone charge employees medical premiums according to their income?
