An employee must notify Human Resources whenever the employee is using a
prescription or over-the-counter drug which may potentially affect
safety or work performance. In deciding whether such a drug may affect
safety and/or work performance, the employee should rely on the warnings
or cautions that accompany that particular legal drug.  The Company does
not seek information on all legal drugs that an employee may be taking,
but only those where there is an indication that the drug may affect
performance, or where there is a caution that one should not engage in
certain activities that are a part of the employee's job duties while
taking the drug.

I will say that you would want to ensure that these statements have been
reviewed by legal counsel to ensure they are okay before implementing.
