[IASHRM] Insurance providers (self insurance???)

I am investigating medical/dental insurance coverage for all of our US based
companies (8 - 1100 employees total).  We have some companies in remote
areas and run into finding uniform coverage that will satisfactorily replace
their current coverage/cost.   We are working with Holmes Murphy, but I'd
like to hear from someone who has worked through this before.  If you'd be
willing to share your insurance provider contact information and why you
went with them that would be appreciated.  Additionally, if you've been with
them for a couple of years, have they been able to control your costs year
to year?   Finally, are you self insured or not?   We are currently not
self-insured, but I am leaning towards moving in that direction.
