[IASHRM] Short Term Disability Policy

Hey, we are looking at modifying our short term disability policy.  Does
anyone have a policy they are willing to share.  I am curious about
administration, if you administer in-house or out source, # of paid
weeks, if you pay at 100%, does the benefit increase with years of
service, etc...  Any help you could give would be great.  Thank you!
