[IASHRM] Exit Interviews

We utilize Survey Monkey.  This web based survey allows us to create our
own survey.  We send it out electronically.  If the employee does not
have computer access they may come to the HR office and use one of our
computers or we can conduct the interview over the phone and then go
into the survey and input the answers.  Survey Monkey allows us to
analyze the data in may different ways.  It is user friendly, however we
do not have a very high response rate.
