[IASHRM] Iowa Law regarding authorization to mail paychecks

Here's the form we developed (from the workforce development website I
believe).  We require direct deposit, but have an intern this summer
who's hometown bank in MN doesn't accommodate direct deposit, so we had
to accommodate.
