[IASHRM] PTO policy

We are considering combining our vacation, sick pay, and personal days
into one PTO policy.  As we draft this new plan many questions have come
up: do we impose a cap or max accrual, at termination do we pay out the
entire balance or just a percentage, do you allow someone to go "in the
hole" if they've used all PTO but get sick and can't work, etc.  If
you're willing to discuss your company's PTO experiences or provide a
copy of your policy it would be greatly appreciated.
