[IASHRM] Bonus Eligibility

We are reviewing our annual bonus eligibility criteria and creating a written plan.  Does anyone have a written plan that is provided to employees regarding participation and eligibility for annual company bonus payments?

For example, we do not pro-rate or pay bonuses to employees who separate from the company before our plan year end.  I am interested in how other companies handle employees who transfer position mid-year or any other issues that I should be taking into consideration.  I understand that criteria is specific to each organization, but we currently do not have anything in writing regarding our past practice and I am looking for templates to help me with this process.

Any advice/assistance would be great appreciated.
