We do not have a use it or loose it provision in our policy. Our
employees continue to accrue PTO. They are not forced to use it or cash
it in however they may cash in or donate PTO if they choose to under the
guidelines listed below. I have attached the form they fill out. We
download all of our employee's banked hours as of the last pay period in
December and keep a database to refer back to through out the year.
They may only buy back up to the amount in their current bank if it is
less than what was in the bank on 12/31 of the prior year. I have
attached a copy of our request form.
CASH IN:
No hours may be cashed in during the employee's first year in the
program. An employee may cash in hours that were available as of
December 31 of the previous year. An employee may request cash a
maximum of three (3) times during the calendar year. Notification must
be in writing and turned into Human Resources. The request must be
received in Human Resources by noon, Friday, of Payroll End to be
available on the paycheck the following Thursday. The appropriate
federal and state deductions will be withheld.
DONATION:
A Great River Medical Center employee, who has available PTO hours in
his/her PTO "bank" and has completed his/her probationary period, is
eligible to donate some of his/her "banked" PTO to another employee who
is experiencing an injury or illness that occurred outside of work or
other major family crisis.
An employee who wishes to receive PTO donations from other employees may
do so by meeting with his/her department director, to let them know that
they are submitting a request to the Vice President of Human Resources
and/or Director of Human Resources for consideration. If approved, the
Human Resources department will then place an electronic message out to
all department directors asking them to post a notice that the employee
wishes to receive PTO donations. Human resources will verify that the
employee wants to receive donations and understands the donation
process, to include the tax implications.
An employee who wishes to donate PTO may do so by completing a PTO
Donation form available in the Human Resources department or by
completing the form on the Human Resources internal website. PTO can
only be donated in four (4) hour increments. The number of PTO hours
donated will be converted into an "Actual Cash Value Equivalent"
utilizing the donating employee's base rate of pay. It will then be
converted into PTO hours by dividing the "Actual Cash Value Equivalent"
by the receiving employee's hourly base rate of pay.
